The health of our wonderful Pharaoh Hounds is always foremost in the minds of the members of the the PHCA.  The Health Survey is one tool we use to assess the health of the population.  Members of PHCA, through the Code of Ethics, have agreed to complete each survey.  However, we are interested in the health of the total breed population, not just those that belong to our membership.  All owners of Pharaoh Hounds are invited and encouraged to complete this survey.

The ELECTRONIC Health Survey is now ready for access! There is an introduction page when the survey is opened explaining what to do. Each single pass through the survey is for only ONE dog in your household. You will have to go back and enter each dog in YOUR household that is currently living with you or that is deceased in the past year. This type of data gathering will provide more accurate information for the breed. It will take some time for those folks with multiple dog households to get your information entered in the beginning, so just take your time. The survey itself is very easy to do.

For those members who do not have computer access please contact me by phone or mail and I will send you our paper version to complete. It will be a version similar to the old survey. Taking the health survey is a PHCA requirement to maintaining good standing with the club.

At the end of the survey you will be asked to enter your name. This last page is NOT linked to the survey you just completed so the information cannot be linked. The last page is to record which members are taking the survey only. OFA is tabulating all data. Please be assured that this is highly confidential.
